If they work the same, why choose one over the other?
Walk into any med spa in Northern Virginia and you’ll likely be offered Botox. Walk into BeLite Medical Center and you’ll be offered Xeomin. That’s a deliberate choice, and patients ask us about it almost every week. The short answer: the two products deliver comparable results in head-to-head testing, but they’re built differently — and we think the difference matters most for the way people actually use these treatments: repeatedly, over many years.
The same active molecule, packaged differently
Botox (onabotulinumtoxinA) and Xeomin (incobotulinumtoxinA) are both FDA-approved botulinum toxin type A products. Both work by relaxing the small facial muscles whose repeated contractions crease the skin of the forehead, the area between the brows, and the corners of the eyes. The difference is what surrounds the active molecule. Botox contains the neurotoxin bundled with accessory proteins (also called complexing proteins). Xeomin contains only the purified 150-kDa neurotoxin — which is why it’s sometimes called the “naked” neurotoxin.
Those accessory proteins don’t contribute to smoothing your skin. Once injected, they separate from the active molecule and play no role in the result. So what are they doing? That’s exactly the question that led us to Xeomin.
What the head-to-head trial showed
This isn’t a marketing comparison — the two products have been tested directly against each other. In a randomized, rater- and patient-blind Phase III trial published in Dermatologic Surgery (Sattler and colleagues, 2010), 381 patients received either 24 units of Xeomin or 24 units of Botox for frown lines between the brows. Four weeks later, independent raters judged 96.4% of Xeomin patients and 95.7% of Botox patients as responders — statistically confirming that Xeomin is non-inferior, unit for unit. Adverse event rates were low in both groups. (Worth knowing: the study was funded by Merz, Xeomin’s manufacturer — but it was independently rated, registered at ClinicalTrials.gov, and its finding of comparable effectiveness is consistent with the rest of the published literature.)
Practical translation: results appear over 3–7 days, last 3–4 months, and dosing converts one-to-one. If you’ve had Botox elsewhere, switching to Xeomin is seamless.
The accessory-protein question: antibody resistance
Here’s why the “naked” formulation matters to us. Any injected protein can, in principle, trigger the immune system to form antibodies — and in a small minority of long-term neurotoxin patients, neutralizing antibodies develop and treatments gradually stop working. Reviews of the clinical evidence have implicated the accessory proteins as a contributor to that immune response, and in Xeomin’s registration trials, no patient developed neutralizing antibodies. To be straightforward about the state of the evidence: this is a mechanistic rationale supported by trial observations, not proof that Xeomin patients will never develop resistance. But when the results are otherwise comparable, we’d rather our patients get the formulation with less unnecessary protein — because cosmetic neurotoxin treatment isn’t a one-time event. It’s something most patients repeat two to four times a year, for years.
Other practical differences
Storage: Xeomin can be stored at room temperature before reconstitution; Botox requires refrigeration. This matters for supply-chain integrity — one less way for product to be mishandled before it reaches you.
Price: because of how the products are made and sold, Xeomin is often more economical — savings we pass on at $12.50 per unit.
Track record: Botox has the longest history and the biggest name. That’s real, and it’s why the comparison question comes up. But “best known” and “best choice” aren’t automatically the same thing.
FAQ
Is Xeomin as strong as Botox? Unit for unit, yes — the head-to-head trial used the same 24-unit dose of each and found comparable response rates.
Will switching from Botox to Xeomin feel different? Most patients notice no difference in the result. Onset (3–7 days) and duration (3–4 months) are comparable.
Why don’t more med spas offer Xeomin? Brand recognition drives demand, and many practices simply stock what patients ask for by name. As a medical practice, we chose based on the formulation instead.
