The Wegovy Pill: Oral Semaglutide for Weight Loss
The Wegovy pill is once-daily oral semaglutide, approved by the FDA on December 22, 2025 as the first GLP-1 tablet for weight management. It delivers weight loss in the same range as the weekly Wegovy injection, without the injection. BeLite began offering it to patients in 2026.
What it is
The tablet contains semaglutide, the same active ingredient as injectable Wegovy, combined with an absorption enhancer so the peptide survives the stomach. Because far less of an oral dose is absorbed, the tablet strengths (1.5 mg, 4 mg, 9 mg, and 25 mg) look nothing like the injection doses; a 25 mg tablet is roughly equivalent to the 2.4 mg weekly shot. It is approved for adults with a BMI of 30 or higher, or 27 or higher with a weight-related condition, and, like the injection, for reducing the risk of major cardiovascular events in adults with heart disease.
What the trial showed
In the 64-week OASIS 4 trial, adults on oral semaglutide 25 mg lost an average of 13.6% of body weight compared with 2.2% on placebo; among those who stayed on treatment the whole time, average loss was 16.6%, and about one in three lost 20% or more. Those numbers are essentially the same as the injection’s STEP 1 results, which is what makes this pill different from the older oral GLP-1 Rybelsus (a diabetes drug dosed far lower).
How it is taken — the rules matter
Oral semaglutide is fragile, so the tablet only works if it is taken exactly as directed:
- First thing in the morning, on an empty stomach.
- Swallowed whole with no more than 4 ounces (half a cup) of plain water. Do not split, crush, or chew it.
- Then wait at least 30 minutes before eating, drinking anything else, or taking any other pill or supplement.
- Start at 1.5 mg and step up roughly every 30 days to 4 mg, 9 mg, and finally 25 mg, as tolerated.
- If you miss a morning dose, skip it and take the next one the following morning; do not double up.
Patients who cannot keep to a fasting morning routine usually do better on the injection or on Foundayo, which has no food or water restrictions.
Side effects and cautions
The side-effect profile matches the injection: nausea, vomiting, diarrhea, constipation, and reduced appetite, mostly early and after dose increases. The same warnings apply: no use with a personal or family history of medullary thyroid carcinoma or MEN 2, not combined with other GLP-1 medications, not used in pregnancy, and a same-day call to your provider for severe or persistent abdominal pain. Because the pill is taken on an empty stomach and delays stomach emptying, timing of other oral medications, especially thyroid medication, should be reviewed with your provider.
Wegovy pill or Wegovy injection?
Same drug, same expected result, different routine. The injection is once a week and forgiving about timing; the pill is every morning with a 30-minute fast. Cost and insurance coverage differ and change often, so we go over current pricing at your visit rather than posting numbers that go stale. Many BeLite patients start on whichever form their coverage favors and switch later if the routine does not suit them.
