Foundayo: The Once-Daily GLP-1 Pill
Foundayo is orforglipron, a once-daily weight-loss tablet from Eli Lilly that the FDA approved on April 1, 2026. It is the first GLP-1 medication that is a small molecule rather than a peptide, which is why it can be taken at any time of day, with or without food, and with as much water as you like. BeLite prescribes Foundayo for patients who want a pill with a simple routine and for patients maintaining weight they have already lost.
What Foundayo is
Orforglipron activates the same GLP-1 receptor as semaglutide, reducing hunger and slowing stomach emptying, but it is built from a different kind of chemistry. Peptide drugs like semaglutide are destroyed by stomach acid, which is why the Wegovy pill needs an empty stomach and a 30-minute wait. Orforglipron is not, so there are no such rules. Foundayo is approved for adults with a BMI of 30 or higher, or 27 or higher with a weight-related condition.
What the trial showed
In the 72-week ATTAIN-1 trial, adults without diabetes on the highest dose lost an average of 12.4% of body weight if they stayed on treatment (11.1% counting everyone who started), versus about 2% on placebo. Blood pressure, triglycerides, and non-HDL cholesterol also improved. That is a smaller result than the injectables: Zepbound reached about 21% and Wegovy about 15% in their own trials. Foundayo is an effective weight-loss medication, though not the most efficacious one. Our post Oral GLP-1 Therapy: What Foundayo Changes goes deeper into the data.
Where it fits
- Patients who want a needle-free option and do not want to manage a fasting morning routine.
- Patients with a moderate amount of weight to lose, for whom 10–12% is the goal.
- Patients stepping down from Zepbound or Wegovy after reaching their target. The ATTAIN-MAINTAIN trial showed orforglipron can hold most of the weight lost on injectables; see Orforglipron Maintenance After Wegovy or Zepbound.
- Patients whose insurance covers it when injectables are not covered.
How it is taken
One tablet once a day, at any time, with or without food. Treatment starts at 0.8 mg and can be increased through 2.5, 5.5, 9, and 14.5 mg to a maximum of 17.2 mg, with each step usually held for at least four weeks. Swallow the tablet whole. If you miss a dose, take the next one at the usual time; do not double up.
Side effects and cautions
Digestive side effects — nausea, constipation, diarrhea, vomiting — are the most common and, as with every GLP-1, are worst in the first weeks and after each dose increase. Foundayo carries the class boxed warning about thyroid C-cell tumors and should not be used by anyone with a personal or family history of medullary thyroid carcinoma or MEN 2. It is not combined with other GLP-1 medications and is not used in pregnancy. Because it is new, long-term safety data are still accumulating; your BeLite provider will review any updates with you.
